There is a small detaunt that the blade sets in so it is centered to the shaft and the bolt. Tightening the bolt with a wrench will not get it tight enough. Make sure the bolt threads are ok and not stripped. See if it's moving with the 3PH. If it is, trace it to under the tractor (if you can with the deck mounted - otherwise you may need to remove it to check the rest of the linkage). That's all I can think of off the top of my head.
